On Thursday, March 9 2023 at approximately 8:40 am, Haverstraw dispatch received a 911 call of an unconscious male at KJ Tile located at 500 Beach Rd in West Haverstraw.
Upon arrival Officers found Carlos Ramirez-Hernandez 52 years old of Spring Valley laying on the floor unresponsive in the rear of the factory surrounded by broken boxes of tile.
He was pronounced deceased on scene by paramedics.
A preliminary investigation revealed that Ramirez-Hernandez was operating a fork lift and reversed into commercial metal shelving containing pallets of tile.
The impact of the forklift striking the shelving caused boxes of tile to fall from the shelving and struck Ramirez-Hernandez.
This incident is being investigated by the Town of Haverstraw Detective Division, Rockland County Sheriff’s Department, Town of Haverstraw Building Department, and the Occupational Safety and Health Administration (OSHA).
